India is transitioning from simply proving AI can read medical data to integrating AI as a trusted, governed, population-scale component of healthcare delivery for over a billion people.
A key strategic model expands specialist capabilities in underserved areas by routing data to trained AI systems that flag high-risk cases for human review, rather than replacing clinicians.
AI applications are expanding to disease prevention and public health, as seen in the Cough Against TB initiative, which screened over 162,000 people and yielded an additional 12–16% TB case detection in deployed settings, guiding targeted screening in high-risk areas.
The biggest challenges include building trust, protecting privacy and data quality, managing model drift and updates, and establishing a durable assurance layer that documents purpose, validation, performance, limitations, human oversight, and post-deployment monitoring.

Regulatory questions focus on how to authorize evolving AI models, given that software can change post-deployment, necessitating continuous validation, monitoring, and re-evaluation rather than a one-time approval.
India’s advantage lies in its scale of health identities, records, facilities, and the AB-PMJAY programme, potentially enabling it to pioneer responsible, population-scale healthcare AI rather than merely adopting technologies from elsewhere.
